Digitalis toxicity:
Features:

  • Generally unwell & lethargy.
  • Nausea & vomiting.
  • Confusion.
  • Yellow-green vision.
  • Arrhythmias (e.g. AV block, bradycardia)
  • Dizziness.

Precipitating factors:

  • Renal disease
  • Hypokalemia
  • Hypomagnesemia
  • Hypoalbuminemia
  • Hypothermia
  • Hypothyroidism
  • Hypercalcemia.
  • Hypernatremia
  • Acidosis.
  • Myocardial ischaemia.
  • Partial AV block.

Drugs:

  • Amiodarone.
  • Quinidine.
  • Verapamil.
  • Spironolactone.
  • Furosemide.
  • Hydrochlorothiazide – Compete with DCT secretion, hence reducing excretion.

Management:

  • Digibind.
  • Correct ventricular arrhythmia by lignocaine.
  • Bradyarrhythmias by propranolol.
  • Atrial tachyarrhythmias by atropine.
  • Phenytoin.
  • Monitor K+
