Digitalis toxicity:
Features:
- Generally unwell & lethargy.
- Nausea & vomiting.
- Confusion.
- Yellow-green vision.
- Arrhythmias (e.g. AV block, bradycardia)
- Dizziness.
Precipitating factors:
- Renal disease
- Hypokalemia
- Hypomagnesemia
- Hypoalbuminemia
- Hypothermia
- Hypothyroidism
- Hypercalcemia.
- Hypernatremia
- Acidosis.
- Myocardial ischaemia.
- Partial AV block.
Drugs:
- Amiodarone.
- Quinidine.
- Verapamil.
- Spironolactone.
- Furosemide.
- Hydrochlorothiazide – Compete with DCT secretion, hence reducing excretion.
Management:
- Digibind.
- Correct ventricular arrhythmia by lignocaine.
- Bradyarrhythmias by propranolol.
- Atrial tachyarrhythmias by atropine.
- Phenytoin.
- Monitor K+
